The Happy Forms plugin is a form plugin for WordPress, it uses a layout similar to the of themes to build forms, so if you are used to working with this tool it will be quite intuitive.
You can customize colors of the different elements, in the free version you have limited fields to be used in the form.
The free version does not store shipment data in the database.
