Thanks for the advice, the first form solved it very easily, I don’t know if it’s because there are only 2 tags to insert like your subject or your message , which are the fields that any email requires.
I have created the form in question very easily with this plugin, the problem is that when inserting the label your message in all the text areas, when giving the submit button, the captcha does not recognize it, apparently it creates a conflict. I erased all the labels I placed and it worked again.
I don’t know if all the labels that exist to place are these or if there are any more.

Being a necessary form for my website and required by the law of my country, what I would need is that each choice of radio buttons, date or writing that the client makes appears in the information that arrives in the mail. Or it occurs to me, it could be an alternative that all the completed information arrives in the mail in the form of a file, but I lack knowledge as advanced as that. Maybe they are simple things but I don’t know how to express it.
I enclose the screenshots of the form in 3 parts, this is as it is at this moment.
